South King County senior care: what’s different

Senior care in South King County — the cities along the I-5 corridor from Federal Way north through Auburn, Kent, Des Moines, Renton, Burien, and SeaTac — looks similar to Pierce County on the surface but runs on a different set of agencies, a different mix of hospitals, and a different AFH market. This post walks through what’s genuinely different about senior placement in South King County compared to Pierce County.
Different Area Agency on Aging
Pierce County families use Pierce County Aging & Disability Resources (ADR) at (253) 798-4600, based in Tacoma. South King County families use Seattle/King County Aging and Disability Services (ADS), the designated Area Agency on Aging for all of King County, headquartered in downtown Seattle at 700 5th Ave Suite 5800 — phone (206) 386-1001. For general information, the Community Living Connections line at 1-844-348-5464 handles intake across the King County service area.
Both AAAs coordinate LTSS planning, caregiver support, and resource referrals, but they run different networks of contracted providers and have somewhat different programming. Families whose parent moves across the county line will sometimes need to re-enroll in county-specific services.
Different DSHS HCS office
Pierce County long-term care Medicaid applications go through the DSHS HCS Tacoma office at 1949 S State St, (253) 476-7200. South King County applications go through the DSHS HCS Federal Way office at 33810 Weyerhaeuser Way S, Suite 201, (206) 341-7600.
Both offices handle CARE assessments, COPES enrollment, and HCS case manager assignments. Processing times and case manager caseloads can differ. Families at the county line (Federal Way specifically straddles the border) sometimes end up assigned to the less-convenient office based on legal residence.
Different hospital networks
Pierce County’s primary hospitals are MultiCare and Virginia Mason Franciscan Health. South King County hospitals include:
- Valley Medical Center (400 S 43rd St, Renton) — Level III trauma, serves Renton, Kent, Tukwila, Auburn, Federal Way.
- MultiCare Auburn Medical Center (202 N Division St, Auburn) — Level 2 Stroke Center.
- St. Francis Hospital (34515 9th Ave S, Federal Way) — Virginia Mason Franciscan Health.
- St. Anne Hospital (16251 Sylvester Rd SW, Burien) — formerly Highline Medical Center, now part of VMFH.
Discharge planners at each hospital maintain relationships with different AFHs and ALFs. A family whose parent was admitted to Valley Medical Center will see different placement recommendations than one whose parent was at MultiCare Tacoma General — not because one list is better, but because local networks differ.
Different AFH market
South King County’s AFH market is somewhat smaller per capita than Pierce County’s, with a higher concentration of ALF memory care communities. Pricing is comparable — private-pay AFH rates run $6,000–$10,000/month, similar to Pierce County — though the geographic distribution is different. Kent and Auburn have the largest AFH density; SeaTac and Burien have smaller markets.
Cultural diversity is a frequent variable. Kent in particular has significant Spanish, Punjabi, Somali, and Russian-speaking populations — and caregiver language match is often a meaningful factor in AFH selection for residents with dementia. Pierce County AFHs generally have less language diversity among caregivers.
Different commute considerations
South King County’s I-5 and SR-167 corridors carry heavy commuter traffic. A family living in Federal Way visiting a parent placed in Auburn (10 miles) often faces 30+ minute commutes during peak hours. Visit timing and route planning matter more here than in the more compact Tacoma market.
Medicaid-path placements
Medicaid-accepting AFHs are proportionally fewer in South King County than in Pierce County — partly because rent and operating costs are higher north of the Pierce line. Families on a Medicaid path often find their best options along the Auburn–Kent corridor rather than in the denser Renton/Burien/SeaTac area.
Crossing the county line
Families living in Federal Way who choose an AFH in Tacoma (or vice versa) are common. Legal residence determines which DSHS HCS office handles Medicaid eligibility, but the home itself doesn’t have to be in the same county as the family. For many families, the best home at the right price is just across the county line.
